Community Health Assessment
Community health assessment is a systematic examination of the health status indicators for a given population that is used to identify key problems and assets in a community. The ultimate goal of a community health assessment is to develop strategies to address the community’s health needs and identified issues. A variety of tools and processed may be used to conduct a community health assessment; the essential ingredients are community engagement and collaborative participation. (PHA Board)

Kansas Health Matters brings community health-related statistical data, local resources and a wealth of information to one, accessible, user-friendly location. Kansas Information for Communities or KIC is a health information portal. You can query public health datasets, access statistics for Kansas counties, and find links to public health reports. KIC is maintained by the Kansas Department of Health and Environment. LEARN MORE
County Health Rankings & Roadmaps
The annual County Health Rankings measure vital health factors, including high school graduation rates, obesity, smoking, unemployment, access to healthy foods, the quality of air and water, income inequality, and teen births in nearly every county in America. The County Health Rankings & Roadmaps program is a collaboration between the Robert Wood Johnson Foundation and the University of Wisconsin Population Health Institute. The Kansas Health Institute has developed individualized County Health Rankings data sheets for each Kansas county, which are available here.

Community Health Improvement Plan
A community health improvement plan is a long-term, systematic effort to address public health problems on the basis of the results of community health assessment activities and the community health improvement process. This plan is used by health and other governmental education and human service agencies, in collaboration with community partners, to set priorities and coordinate and target resources. A community health improvement plan is critical for developing policies and defining actions to target efforts that promote health. It should define the vision for the health of the community through a collaborative process and should address the gamut of strengths, weaknesses, challenges, and opportunities that exist in the community to improve the health status of that community. Emergency Operations Plan
An emergency operations plan is a document that assigns responsibility to organizations and individuals for carrying out specific actions at projected times and places in an emergency that exceeds the capability or routine responsibility of any one agency; sets forth lines of authority and organizational relationships, and shows how all actions will be coordinated; describes how people and property will be protected in emergencies and disasters; identifies personnel, equipment, facilities, supplies, and other resources available–within the jurisdiction or by agreement with other jurisdictions–for use during response and recovery operations; and identifies steps to address mitigation concerns during response and recovery activities. As a public document, an EOP also cites its legal basis, states its objectives, and acknowledges assumptions. (FEMA, p 14)

Health in all Policies (HIAP)
HiAP is a collaborative approach that integrates and articulates health considerations into policy making across sectors, and at all levels, to improve the health of all communities and people.[1] There are various ways to implement HiAP, including convening a cross-sector coalition or task force, creating and/or using a data dashboard that includes indicators and objectives from various sectors, and projecting health impacts of policies.

Strategic Plan
The Public Health Accreditation Board (PHAB) defines strategic planning as “a disciplined process aimed at producing fundamental decisions and actions that will shape and guide what an organization is, what it does, and why it does what it does. The process of assessing a changing environment to create a vision of the future; determining how the organization fits into the anticipated environment, based on its mission, strengths, and weaknesses; then setting in motion a plan of action to position the organization,” with the strategic plan focusing on a range of agency level organizational goals, strategies and objectives, including new initiatives. Workforce Development Plan
Measure 8.2.1A Health departments must have a competent workforce with the skills and experience needed to perform their duties and carry out the health department’s mission. Workforce development strategies support the health department, individual staff members, staff development, and the overall workplace environment. Quality Improvement Plan
The QI Plan is a guidance document that informs everyone in the organization as to the direction, timeline, activities, and importance of quality and QI in the organization. The QI Plan is a living document and needs to be revised on a regular basis to reflect accomplishments, lessons learned, and changing organizational priorities. It is not a one-time static document but one that should constantly describe the current state and future state of quality in any public health department. (Public Health Foundation)

Performance Management System
Performance management encompasses the ongoing process of measuring, monitoring, and reporting of progress toward strategic organization, division, and program goals and objectives.1Performance management is closely linked with quality improvement (QI), as it provides a structured, data-driven approach to identifying and prioritizing opportunities for improvement. However, performance management is distinct from quality improvement. Performance management is the continuous use of four components: performance standards, performance measures, reporting of progress, and quality improvement. (NACCHO)

The 10 Essential Public Health Services provide a framework for public health to protect and promote the health of all people in all communities. To achieve equity, the Essential Public Health Services actively promote policies, systems, and overall community conditions that enable optimal health for all and seek to remove systemic and structural barriers that have resulted in health inequities. Such barriers include poverty, racism, gender discrimination, ableism, and other forms of oppression. Everyone should have a fair and just opportunity to achieve optimal health and well-being.
